In times of uncertainty, we want you to know that safety is our highest priority regarding the health of our patients, employees, and families.
We have re-opened our office effective May 4th, 2020 but with many restrictions to protect our patients and staff.
All patient forms are now located on our website. You can even enter your drivers license and insurance information prior to your arrival (You can also snap a picture of them and send to us). All forms must be filled out online- we cannot see anyone who has not done so. No paper forms will be accepted until restrictons have been lifted. We also now offer online payments for services.
Vision Quest Optical is also open on an appointment basis. Call 972-428-0236 to set up a time.
Here are our strict guidelines:
We have installed HEPA filtration units throughout the office. In addition, we have adopted a strong social distancing policy and cleanliness policy. All patients and all staff will be required to wear facemasks and/or shields and in some cases sterile gloves. We ask that patients bring their own facemasks to the office. Upon entering our office you will be immediately taken to a hand washing station.
If you or a family member have had any of these we will ask you to re-schedule:
- Fever >100 degrees, cough, trouble breathing?
- Cold and/or flu symptoms such as sneezing, runny nose, sore throat, body aches?
- Body rash with fever?
- Traveled outside the country?
- Were you tested positive for COVID 19
- Have you been in contact with a person who contracted COVID 19
All of our staff also has to take this "test" daily.
